Student “In Case of Emergency Card”, don’t go to college without it.

This is a come and go event for college bound students and their parents to get information about why it is important for students to execute a HIPAA waiver to insure that doctors and health care providers know that they can share medical information.  The card provides emergency contact information, allergies and medical conditions that may be vital in in an emergency.  It also provides doctors with the ability to access the releases and other information.

There is no fee for attending or getting additional information.  If the student elects to participate a fee for completing the documents and for membership in DocuBank will charged at that time.
