Here are important forms that are used to apply for Wartime Veteran’s Benefits.

Note:  This is to be used by Veterans and their families only.  These are forms and general instructions for those who believe that they are financially qualified to receive benefits.  Generally, the Veteran must have less than $80,000 in countable assets to be eligible.  Veterans who’s assets exceed this amount should seek the advice of a VA accredited attorney prior to applying.  These materials are not a comprehensive or exhaustive instruction manual.  Applicants are encouraged to conduct additional research.

  • Request for Military Records (DD214) This is to be used if you do not have the service records.  Standard Form 180
